  • Overview

    The Care Management Supervisor is responsible for daily coordination and support of the various care management functions across the continuum. The position serves as a resource to staff and other departments regarding all aspects of the Care Management department. This position assists the Executive Director of Care Management with hiring and training of staff and managing daily department operations and collaborates with staff in promoting department performance improvement initiatives.

     

    Provides patient care, treatment, and services within the scope of their license, certification, registration, and/or assessed competencies.  Practice will be in accordance with laws and regulations and will adhere to established policies, procedures and practice guidelines.

    Responsibilities

     

    1. Acts as a resource to staff and ancillary departments regarding all aspects of the Care Management department’s processes for discharge planning, care coordination, utilization management, insurance plan requirements and state and federal regulations.
    2. Under direction of the Executive Director of Care Management, manages daily department clinical operations by supporting adequate staff coverage, workload assignments, and productivity standards.
    3. Coordinates educational activities and orientation program for Care Management staff and assists in the development and monitoring of performance improvement activities and staff development.
    4. Collaborates with the Executive Director of Care Management, and other departments concerning data collection, analysis, reporting and performance improvement initiatives.
    5. As part of the management team, participates in strategic planning, customer service initiatives and recovery, complaint resolution, physician relations, and representing the department on hospital-wide committees.
    6. Promotes department performance improvement initiatives such as meeting productivity standards, quality improvement, customer service excellence, and length of stay.
    7. Participates in continuous quality improvement teams and activities.
    8. Participates in supporting the organization’s vision, mission and values and adheres to DeKalb Medical Standards of Behavior.
    9. Performs other duties as assigned to meet the goals and objectives of DeKalb Medical.
    10. Supervises departmental staff within established budget to facilitate effective services as follows:

     

    • Recruits, selects, trains, and supervises departmental staff. Evaluates performance, making recommendations for personnel actions and motivation of employees to achieve superior performance.
    • Oversees time and attendance coordination for assigned staff, including monitoring overtime and productivity.
    • Assists in the development and monitoring of departmental budget. Ensures appropriate provision and utilization of departmental resources.
    • Promotes and facilitates continuous quality improvement activities to ensure the provision of and improvement upon safe and quality patient care.

     11.Performs other duties as assigned to meet the goals and objectives of DeKalb Medical.

     

    REPORTING RELATIONSHIPS:

    Reports to: Executive Director of Care Management

    Positions Supervised: Care Management clinical, social work, and  support staff

    Qualifications

    Minimum Education, Experience and Licensure Required:

    • Bachelor’s degree in Nursing (BSN) or Master’s degree in Social Work (MSW) required
    • Graduate of an accredited school of Nursing or Social Work
    • Minimum of five (5) years of comprehensive hospital case management experience required
    • Current GA Licensure as a Licensed Clinical Social Worker (LCSW) or as a Registered Nurse (RN)
    • Prior Case Management leadership experience preferred
    • Demonstrated experience and interest in care quality and resource utilization improvements required
