Responsible for direct services to adult patients and significant others through individual, family and group psychotherapy and individual bio-psychosocial assessments. This staff member is also responsible for enhancing quality improvement, performance improvement initiatives, building community relationships, assisting with clinical assessments and discharge planning as needed, and supporting the Director in advancing department and hospital goals.
Provides patient care, treatment, and services within the scope of their license, certification, registration, and/or assessed competencies. Practice will be in accordance with laws and regulations and will adhere to established policies, procedures and practice guidelines.
Analyzes patient data provided by multidisciplinary team and clinical assessment and updates treatment plan in a timely fashion as defined by hospital, provider, and JC standards.Attends in-service education programs treatment planning with multidisciplinary team.
Plans and provides individual and group education as assigned.
Maintains a thorough knowledge of hospital’s administrative operations and a familiarity with all aspects of treatment services for providing patient care.
Recognizes and reports abnormalities or significant changes in patient’s condition promptly to appropriate individuals (physicians, supervisor, nursing staff, nutritionist, family therapist, consulting psychologist and psychiatrist).
Coordinates and organizes care of assigned patients based on patient’s needs.Insures completion and/or readiness of chart (treatment needs assessments, treatment plans, staffing and discharge plans).
Participates in referral development and community relations as appropriate and other teams as designated.
Identifies teaching/learning needs of the assigned patient and/or significant others.
Develops and updates treatment plan as patient’s conditions warrants (in areas required) and reviews with the patient.
Maintains vigilance over patient insuring necessary safety precautions are taken. Identifies high risk patient/visitors.
Applies the principles of safety in a therapeutic environment. Can assess for suicidal/homicidal ideation and initiate appropriate contract.
Facilitates inpatient group and individual psychotherapy and psycho-education groups effectively.
Recognizes legal/ethical principles related to staff code of ethics.
Keeps supervisor aware of any noncompliance or potential risk management problems.
Deals with problems by actively seeking solutions in a positive manner.
Asserts self to uncover opportunities for improvement in the service of both internal and external customers.
Nurtures relationships with others that result in trust, team work, and effective communication.
Participates in continuous quality improvement teams and activities.
